OFFICIAL RULES

NO PURCHASE OR PAYMENT IS NECESSARY TO ENTER OR WIN.

1. ELIGIBILITY. Contest is open to New York area residents 18 years of age or older.  Officers, directors and agents of Continuum Hospitals of New York  (“Sponsor”), and its subsidiaries, parent organizations, affiliates, service agencies, independent contractors, advertising/sales promotion agencies, and judging organization (“Sweepstakes Entities”), and the immediate family members (spouse, parents, siblings, children and in-laws) and those living in the same household of each such individual are not eligible.  Void where prohibited.

2. ENTRY. Contest begins at 12:00 am EST April 1, 2013 and ends at 11:59 pm EST April 30, 2013 ("Promotion Period").  To enter, you must first complete a registration form on the Live Well New York website.  After doing so, you will receive one (1) entry into the Contest.  Limit: one (1) entry per person.  Sponsor is not responsible for technical or other errors or malfunctions in the transmitting or processing of online entries. CAUTION:  Any attempt by an entrant to deliberately undermine the legitimate operations of the contest is a violation of the criminal and civil laws.  Should such an attempt be made, the Sponsor reserves the right to seek damages from any such entrant to the fullest extent permitted by law and to disqualify such entrant from the Contest.  

   Winner will be selected on April 7, 2013 in a random drawing from all eligible entries received, to be conducted by an independent judging organization whose decisions shall be final.  Odds of winning depend upon the total number of eligible entries received.  The prize will be awarded provided a sufficient number of entries are received and winner will be notified via email within approximately 3 days of the drawing.

3. SWEEPSTAKES PRIZES.  One (1) Grand Prize: Whole Foods Market gift card valued at $250 
   In case of unavailability of any prize, Sponsor reserves the right to substitute a prize of equal or greater value. No cash redemption, transfer or substitution of prize will be permitted except as provided herein.  Failure to claim prize or return of prize notification letter as undeliverable will result in forfeiture of prize, and an alternate winner may be selected. Prize (or any portion thereof) may not be sold, or transferred or given to a third party.

4. CONDITIONS. All federal, state and local laws and regulations apply. Limit one prize per person. By accepting a prize, winner agrees to hold harmless and indemnify Sponsor and Contest Entities against any and all liability, damages or causes of action (however named or described), with respect to or arising out of entrant’s participation in the promotion, receipt, possession, use and/or misuse of any prize awarded herein. These parties, their respective affiliates and subsidiaries, advertising/promotional agencies, and each of their directors, officers, employees, agents, successors and assigns have no liability or responsibility for any personal injuries, losses or damages of any kind resulting from participation in the sweepstakes, acceptance, possession or use/misuse of any prize.    WINNERS ACKNOWLEDGE THAT SPONSOR HAS NOT MADE ANY DISCLAIMS AND ALL LIABILITY OR RESPONSIBILITY FOR ANY WARRANTY, REPRESENTATION OR GUARANTEE, WHETHER EXPRESS OR IMPLIED, IN FACT OR IN LAW, RELATIVE TO PRIZES, INCLUDING, BUT NOT LIMITED TO AUTHENTICITY, QUALITY, MECHANICAL CONDITION, MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE AND AGREE TO LOOK SOLELY TO THE PROVIDERS OF THE PRIZES FOR ANY SUCH WARRANTY AND/OR GUARANTEE.
